A dental insurance policy with a choice of two benefit maximums and a choice of four deductibles. Policy is guaranteed issue.

Persons age 65 and up may apply, based on age last birthday.

Spouse of any individual that qualifies may apply regardless of age.

Guaranteed Issue. Applicant can choose Standard Plan (low benefit )with a $1,000 maximum annual benefit or the Enhanced Plan (high benefit) with benefits 150% of the Standard Plan and a $1,500 maximum annual benefit. Choice of $25.00, $50.00, $75.00 or $100.00 annual deductible.

Type I Services: Routine exam; prophylaxis

Type II Services: X-rays, fillings, simple extractions, palliative treatment

Type III Services: Biopsies, histopathologic examinations; repairs to existing crowns, dentures and bridges; recementation of existing crowns, inlays, onlays, and bridges; therapeutic drug injections

Type IV Services: Endodontics (root canals); non-surgical periodontics (gum treatment); surgical extractions; extraction of impacted teeth; other oral surgery; general anesthesia and IV sedation; denture relines and rebases; denture adjustments

Type V Services: Periodontal Surgery; inlays and onlays; crowns; complete and partial dentures; fixed bridgework
